This virtual presentation will use a series of neurologic patient cases to examine the impact of social determinants of health (SDoH) on patient outcomes in an outpatient setting. The neurologic patient cases will be shared from the Jefferson Moss-Magee Rehab healthcare system, primarily from 2025-2026. Recent research on social determinants of health and patient rehabilitation outcomes will be summarized and applied to the patient cases. Participants will be encouraged to engage in discussion of this real-life impact of SDoH on patient outcomes, and recommendations for promoting equal opportunities in outpatient settings.

Speakers: 

Alia Hurley, PT, DPT. Alia is a 2025-2026 Jefferson Moss Magee Rehabilitation (JMMR) Neurologic Physical Therapy Resident. Alia graduated with her Doctorate in Physical Therapy from Thomas Jefferson University in 2025. She completed clinical rotations at Concentra Urgent Care, Magee Rehabilitation Hospital, and Thomas Jefferson University Hospital. She earned her undergraduate degree at Muhlenberg College with a major in dance science.

Sam Gifford, PT, DPT. Samantha Gifford earned a Bachelor of Science in Clinical Health Studies with a minor in Psychology in 2021 and a Doctorate of Physical Therapy in 2023, both from Ithaca College. Her experience includes work in the outpatient setting at The Rehab Center in Charlotte, North Carolina, as well as inpatient rehabilitation and acute care at Grand Strand Medical Center in Myrtle Beach, South Caroline. She is currently participating in the Jefferson Moss-Magee (JMMR) PT Neurologic Residency Program.

Seamus Moran, PT, DPT. Seamus earned a Bachelor of Science in Exercise Science with minors in Mathematics and Women’s & Gender Studies from West Chester University in 2019. He earned a Doctor of Physical Therapy degree from Arcadia University in 2022. He began working per diem for Bryn Mawr Rehab Hospital (Malvern, PA) and is involved in Arcadia University’s DPT program as a mentor and lab instructor. Seamus is participating in the Jefferson Moss-Magee Rehab (JMMR) PT Neurologic Residency Program.
