Request Contact Hours (APTA Pennsylvania Chapter Components Only)

• Contact Hour Request Form
• This approval form is for APTA Pennsylvania Chapter components ONLY. 
• All external parties must submit all required information to the PA State Board of Physical Therapy.
• Components must complete the CEU request form 90 days prior to the continuing education course (or 30 days prior for 1-2 hour District Meeting continuing education). 
• Remember that you need to send the information to chapter office 30 days in advance for district meetings and 90 for continuing education seminars.

The APTA Pennsylvania only pre-approves courses sponsored through and for the APTA Pennsylvania Districts/Special Interest Groups (SIGs)/Committees. Other CE approval questions will have to be directed to the State Board of Physical Therapy | P.O.
